Care for the Community |
Citizens’ Hose Co. No. 1 - EMS is a professional Basic
Life Support organization. We are staffed by both volunteer and career personnel.
We operate two ambulances that are completely equipped with the most up-to-date
equipment available to service the needs of the community. We strive to provide
a professional and courteous service with a personal touch.
Memberships provide the basic funding for your ambulance service. This funding
ensures the best possible ambulance service that can be provided to you, by
enabling us to maintain or purchase supplies, equipment or vehicles whenever
necessary. In addition, a portion of your donation is also used for the general
operating costs of the Citizens' Hose Company No. 1 EMS.
Memberships begin April 1st, 2005 and end on March 31st, 2006.
OUR GOAL IS TO GIVE BACK TO THE COMMUNITY AND SHOW THAT WE ARE HERE
BECAUSE WE CARE.

Types of Memberships |
Family - $75.00 This includes anyone who permanently resides in
the household.
Individual - $60.00 This is available to an individual (one) person in a residence.
ALS receives thirty dollars ($30.00) from each membership collected. Citizens’
Hose Co. No. 1 - EMS receives the remainder.
| Medicare now mandates that an authorization signature accompany all submissions for reimbursement. Any bill not having one will not be permitted for submission. For this reason, any member or representative, declining or is not available to sign an insurance authorization form for CHC-EMS, the member, representative, or estate may be held responsible for the entire financial obligation of that specific service. This applies regardless of insurance carrier. |

VERY IMPORTANT
NOTICE |
Any transport that is found to be NOT MEDICALLY NECESSARY (meaning that person could have gone by other means), will NOT be covered under the membership. The individual, representative, or estate will be held responsible for payment.

Membership Includes
|
As a member, you will be transported to the hospital of your choice within a 30-mile radius in an emergency. The ALS service responds along with the ambulance to your residence if needed. This unit carries licensed paramedics with their equipment, to render advanced care, (i.e. intravenous fluids, heart monitoring, and drug therapy), in conjunction with the treatment our ambulance crew provides while a patient is being transported to the hospital. Non-Emergency Transports |
We do not provide this service free of charge as part of the membership, unless it is medically necessary for an ambulance to transport you as deemed necessary by Medicare standards. We are still willing to arrange these transports for our members. However, you will be personally responsible for payment in full. It will not be covered under the membership. Our members do, however, receive discounts on these services.

* House markers are available at a small cost. If we can’t
find you-we can’t help you.
* CHC - EMS has CPR/First Aid instructors on staff. We would be happy to assist
you with establishing CPR classes for your church, social group, business or
family.
* We enjoy speaking to students and civic organizations about health and safety
issues, as well as career opportunities.
* We participate with events such as training, health fairs, bike derby, fundraisers,
public education, and several other community oriented events.
